Hope Card Request Instructions
We recommend having a copy of your current, Final Order of Protection (OP) available prior to filling out the Hope Card request form. All the information required to request a hope card can be found on this document. Temporary Orders of Protection are not eligible for the Hope Card program.
Enter the required information in the appropriate fields in the Hope Card request form.
Order of Protection Information
- Select the county where your order was issued
- Select the court that issued your order

Select the form type
- If your Order of Protection (OP) was issued in family court case, select either Family – GF5a or Family – GF6a
NOTE: Your document will say General Form 5a or General Form 6a in the upper right corner - If your Order of Protection (OP) was issued in a matrimonial (divorce) case, select Supreme Matrimonial
NOTE: Your form will say Form SC-2 in the upper right corner - If your Order of Protection (OP) was issued in a criminal case, select either Criminal – Family Offense or Criminal – Non-Family Offense
NOTE: Your form will say Criminal Form 1/CRIM1 or Criminal Form 2/CRIM2 in the upper right corner
- If your Order of Protection (OP) was issued in family court case, select either Family – GF5a or Family – GF6a

Enter the Order Number or Issue and Expiration Dates
- If your form type is Family or Supreme Matrimonial, enter the Order Number. This is a 10-digit number, typically formatted as YYYY-NNNNNN
Note: Your Order Number is located on the top of your OP - If your form type is Criminal Family or Non-Family, enter the Order Number, if known. This is a 10-digit number, typically formatted as YYYY-NNNNNN If you do not know your Order Number, you can enter the Issue Date and Expiration Date
Note: The Order Number is located at the top of your OP
- If your form type is Family or Supreme Matrimonial, enter the Order Number. This is a 10-digit number, typically formatted as YYYY-NNNNNN
- Enter the First and Last name of the protected party (the person the Hope Card is for)
- Enter the Date of Birth of the protected party (the person the Hope Card is for)
- Enter the First and Last name of the party your Final Order of Protection is against

Enter the First and Last name of the person requesting the Hope Card if that person is not the protected party
Note: The following people or agencies are legally authorized to request a Hope Card for the protected party:-
For Family and Supreme Matrimonial:
- Protected Party
- Parent (if the OP is not against this person)
- Guardian (if the OP is not against this person)
- Agency that has custody of a child who is a protected party
- Attorney representing the protected party

For Criminal Family and Non-Family:
- Protected Party
- Prosecutor (District Attorney)
- Crime Victim Advocate/Agency
- Domestic Violence Advocate/Agency

For Family and Supreme Matrimonial:
- Check the "I am human" hCaptcha box
- Click the “Next” button

The Contact Form page shows a summary of the details you shared for the Order of Protection. It also lets you choose how you want to receive your Hope Card.

Select your preferred method(s) of receiving your Hope Card (text, email, and/or physical mail). Provide the requested contact information.
- Phone Number: Enter a valid, US phone number where you can receive text messages. You will receive a link to your digital Hope Card here.
- Email Address: Enter a valid email address where you can receive messages. You will receive a link to your digital Hope Card here.
- Mailing Address: Enter a valid US or Non-US address where you can receive mail. You will receive your physical Hope Card here.
- Check the "I am human" hCaptcha box
- Click the “Submit” button to complete the request
Digital Hope Cards (text or email) are sent within 30 minutes. Physical Hope Cards (by mail) are sent within 10 days.
